First Adult Passport (Ages 16 and older)

Applicants aged 16 and older who is holding 5 years valid passport must apply in person using Form DS-11 and must execute the passport application on their own behalf.

Note: Applicants who are 16 or 17 must also demonstrate one of the following ONE parental awareness of their application.

  • Parent Applies with You: One of your legal parents or guardians applies with you, and signs Form DS-11. Bring a photocopy of that parent or guardian’s ID.
  • Parent Signs Note: Submit a signed note from your legal parent or guardian with a photocopy of their ID.
  • Parent Pays Fees: Submit proof that your legal parent or guardian is paying your fees. Example: a parent or guardian’s name is on electronic payment confirmation
  • Notarized Statement: We may ask you to submit a notarized statement (example: complete Form DS-3053) from your legal parent or guardian which says you can have a passport. Include a photocopy of that parent or guardian’s ID.

Passports for applicants 16 and older are valid for 10 years.

The process may take up to four to six weeks from the date we receive your application to when you receive your new passport book by mail. Please note, there is no expedite service when applying for U.S. passport overseas.

As of April 10, 2023, the Embassy/Consulate will no longer issue passport linkage letters with newly issued passports. If a Thai government office requests one, your cancelled passport and your newly issued passport will suffice. Renewal

Required documents:

1. DS-11: Application for a U.S. Passport

*NOTE: You must provide a Social Security Number if you been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).

  • If you have never been issued, have lost, or have forgotten your Social Security Number, please click hereIf an applicant is unable to provide their SSN, then U.S. Embassy Bangkok can issue a direct-return passport to the United States only.      
  • If you have never been issued Social Security Number (SSN), you must submit a statement, signed and dated, that includes the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct:  I have never been issued a Social Security Number by the Social Security Administration.”

2. Current passport and 1 clear photocopy (valid for 5 years, or was issued more than 15 years from issued date)

3. Fees payment

  • On-line Payment only available in USD
  • Pay the $165 passport application fee via the U.S. government’s secure payment site. When click Continue to the Form; please select Thailand at Country field.
  • After successfully paying the fee, you must print out the electronic payment confirmation you receive via email and present it on your appointment.
  • If you are submitting a payment on behalf of another passport renewal applicant, please enter their name. All fields are required.

Paying online allows you to avoid long wait times during your appointment at the U.S. Embassy/Consulate. However, you may review optional onsite payment methods.

*NOTE: You can only use this form to pay for one passport application at a time. Fee payments for multiple renewal applications must be submitted separately.

4. Passport photo, provide two WHITE background color passport photos, 2” x 2” (5 x 5 cm) taken within the past 6 months; no eyeglasses. See photo requirements

5. Citizenship evidence and 1 clear photocopy

First Time Application

Required documents:

1. DS-11: Application for a U.S. Passport

Name usage: The name on the form DS-11 should be the name recorded on your citizenship evidence such as Naturalization Certificate, U.S. birth certificate, U.S. Consular Report of Birth Abroad Certificate. Should you wish to document your name differently, an official name change document such as Court order of Name change, or Name change certificate must be provided to reflect the change.
