If you have reported your Passport Lost/Stolen, you MUST get it replaced before traveling or re-entry to Thailand.
You must replace your passport if you have reported it lost/stolen, even if you find it. If you arrive in Thailand with a lost/stolen passport, you will not be admitted into the country. How to Replace a Lost/Stolen Adult Passport (Ages 16+)
The loss of a valid U.S. passport is a serious matter. After making every attempt to recover the missing passport, you must obtain a police report from the Thai Police station near the location the loss occurred or nearest you. (Tourist Police Hotline: 1155)

Requirements:
1. Printed Application form DS-11: Application for a U.S. Passport (completed, but NOT signed)
Note: You must provide a Social Security Number if you been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
- If you do not have a Social Security Number (SSN), forgotten or lost it, please click here.
If an applicant is unable to provide their SSN, then U.S. Embassy Bangkok can issue a direct-return passport to the United States only. - If you have never been issued SSN, you must submit a statement, signed and dated, that includes the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security Number by the Social Security Administration.”
2. Original police report: file a report at the nearest police station
3. Printed form DS-64 – Statement Regarding a Valid Lost or Stolen U.S. Passport Book and/or Card (completed, but NOT signed)
4. A photocopy of your lost/stolen passport, if available
5. A valid government issued photo Identification and 1 clear photocopy, if available
6. Application fees payment
- On-line Payment only available in USD
Pay the $165 passport application fee online here U.S. government’s secure payment site. Paying online allows you to avoid long wait times during your appointment at the U.S. Embassy/Consulate. However, you may review optional onsite payment methods.
When paying online you should click Continue to the Form and then please select Thailand in the country field.
Online Payment is only available in USD. After successfully paying the fee, you must print out the electronic payment confirmation you receive via email and present it on your appointment.
If you are submitting payment on behalf of another passport renewal applicant, please enter their name. All fields are required.
NOTE: You can only use this form to pay for one passport application at a time. Fee payments for multiple renewal applications must be submitted separately.
7. Passport photo
Provide two WHITE background color passport photos, 2” x 2” (5 x 5 cm) taken within the past 6 months; no eyeglasses, no selfie and no filter. See more information on photo requirements
