General Information
A minor aged 0-15 who is holding 5 years valid passport must apply in person using Form DS-11. Both parents are also required; If one parent cannot attend please see Optional: Form DS-3053.
Passport process takes approximately four to six weeks. Please note, there is no expedite service when applying for U.S. passport overseas.
As of April 10, 2023, the Embassy/Consulate will no longer issue passport linkage letters with newly issued passports. If a Thai government office requests one, your cancelled passport and your newly issued passport will suffice. You may download this letter (PDF 70KB) in Thai and English that explains the change in Embassy policy.
IMPORTANT: all required documents listed below must be present in original. Missing one of those documents could possibly delay a renewal process.

Renewal
Requirements:
1.The physical presence of the applicant child (Required, NO EXCEPTIONS)
2. The applicant’s current passport and 1 clear photocopy (valid for 5 years, or was issued more than 15 years from issued date
3. Application form DS-11: Application for a U.S. Passport
- * NOTE: You must provide a Social Security Number if you been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f). Only the number is required. The physical Social Security card is not necessary in most situations.
- If you do not have an SSN, have forgotten, or lost your Social Security Number, please click here. If an applicant is unable to provide their SSN, then U.S. Embassy Bangkok can issue a direct-return passport to the United States only.
- If you have never been issued an SSN, you must submit a statement, signed and dated, that includes the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security Number by the Social Security Administration.”
4. Application fees payment
- On-line Payment only available in USD
Pay the $135 passport application fee online here U.S. government’s secure payment site. Paying online allows you to avoid long wait times during your appointment at the U.S. Embassy/Consulate. However, you may review optional onsite payment methods.
When paying online you should click Continue to the Form and then please select Thailand in the country field.
Online Payment is only available in USD. After successfully paying the fee, you must print out the electronic payment confirmation you receive via email and present it on your appointment.
If you are submitting payment on behalf of another passport renewal applicant, please enter their name. All fields are required.
NOTE: You can only use this form to pay for one passport application at a time. Fee payments for multiple renewal applications must be submitted separately.
5. Passport photo
You must provide two photos with your passport application. All our photo policies apply to both adults and children under age 16. We have a few exceptions to our photo policies for infants. See photo requirements
Photo Tips
Acceptable – No other person is in the photo, and the child is facing the camera with eyes open. It is acceptable, however, if your infant’s eyes are not entirely open.

6. Citizenship evidence and 1 clear photocopy
7. Parental Relationship evidence: You must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport. Please note, CRBAs (Consular Report of Birth Abroad) are not acceptable as evidence of legal parentage for minor passport applications.
The following may be used to show parental relationship:
- U.S. Birth Certificate (also considered as U.S. Citizenship evidence)
- Foreign Birth Certificate
- Adoption Decree
- Divorce/Custody Decree
Please note: Some documents, like a U.S. birth certificate, show both U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies). For a foreign language document, please provide both original documents in a local language and a certified English translation.
8. Printed Growth Progression Photos:

A minimum of 1 photo for every year of life from the date the last passport was issued to now. (Example: yearly school or birthday photos. The photos will be returned to you)
9. Both parents and two-parent consent
- Both parent’s valid passport and/or government issued photo ID and 1 clear photocopy
(Proof of legal name change is required if the name on the ID does not match the name on the child’s birth certificate. If the certificate is in any language other than English, you must also provide a certified translation.)
- Two-parent (Parental) Consent, if one parent cannot attend. The non-attending parent may fill out the DS-3053 and bring the original Form DS-3053 .
IMPORTANT: Two-parent consent is required when obtaining or renewing a passport for a minor under age 16. If one parent cannot attend an interview, please select one of the following conditions to see required documents.

First Time Application
Requirements:
1. The physical presence of the applicant child (Required, NO EXCEPTIONS)
2. Citizenship evidence and 1 clear photocopy. If you are applying for a Consular Report of Birth Abroad at the same time as the first passport, please bring the necessary documents as outlined on our “Report the Overseas Birth of a U.S. Citizen Abroad” page.
3. Application form DS-11: Application for a U.S. Passport
Name usage: The name on the form DS-11 should be the name recorded on your citizenship evidence such as Naturalization Certificate, U.S. birth certificate, U.S. Consular Report of Birth Abroad Certificate. Should you wish to document your name differently, an official name change document such as Court order of Name change, or Name change certificate must be provided to reflect the change.
