Data Correction/Information Changes on Passport

If your current U.S. passport requires a correction due to data error or an update to your information, such as a name change, please see related information below.

Data Correction or Printing Errors

You can correct your passport if there is a data or printing errors. A data error may include an issue with your name, gender, or place of birth. A printing error may include data that is missing on the biographical page, discoloration, or crooked printing. We will correct the error at no charge if the passport is still valid.

Note:

  • If you are reporting the error within one year of us issuing your incorrect passport, the new passport will be valid for 10 years.
  • If you are reporting the error after one year, the new passport will be valid until the expiration date of your original, incorrect passport. 

To correct a data or a printing error, please submit the following documents by mail:

  1. DS-5504: Application for a U.S. Passport for Eligible Individuals, Correction, Name Change to Passport Issued 1 year ago or less, and Limited Passport Replacement.

NOTE: You must provide a Social Security number if you been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).

  • If you do not have, forgotten, or lost your Social Security number, please click here. If an applicant is unable to provide their SSN, then U.S. Embassy Bangkok can issue a direct-return passport to the United States only.
  • If you have never been issued Social Security Number (SSN), you must submit a statement, signed and dated, that includes the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct:  I have never been issued a Social Security Number by the Social Security Administration.”

Remarks: For a minor age 0-15, one of parents must sign the application and submit two additional documents:

  • A copy of a valid passport or a government issued photo ID of parent who signs the application,
  • Parental Relationship evidence: You must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport. Please note, CRBAs (Consular Report of Birth Abroad) are not acceptable as evidence of legal parentage for minor passport applications.
    The following may be used to show parental relationship:
    – U.S. Birth Certificate (also considered as U.S. Citizenship evidence)
    – Foreign Birth Certificate
    – Adoption Decree
    – Divorce/Custody Decree
  1. The applicant’s current passport with 1 clear photocopy
  1. Two passport photographs (See photo requirements)
  1. Evidence of the correction (e.g. your U.S. birth certificate showing the correct spelling of your last name, or your previous passport).
  1. Return envelope fee: Pay for Thailand Post Courier Pass back envelope fee 100THB here

    • Sign up before continuing if you are a new user, 
    • Complete all required information, see *asterisks, 
    • Ensures providing Applicant’s full name and accurate returning address on both billing and shipping address fields. 

    Please include a copy of payment confirmation with your application package.  

  1. Mail application: please mail your package to U.S. Embassy Bangkok per the address below via one of the following couriers (Thailand Post and DHL) or else your package will be rejected. Please ensure that you receive a tracking number from the courier for your mailed package.

Passport was issued within one year and a name was legally changed

If your name was legally changed less than one year, you can replace it at no charge by mail if it is not damaged and was issued less than one year ago. Please submit the following documents by mail:

  1. DS-5504: Application for a U.S. Passport for eligible individuals, Correction, Name Change to Passport Issued 1 year ago or less, and limited passport replacement.

Your name was legally changed, and your passport was issued more than one year ago

If you changed your name and your current passport was issued more than one year ago, you may be eligible to renew your passport by mail or in person using the appropriate form.

You have used a different name on other identification cards, but cannot show a legal name change

If you cannot show the change with a court order, divorce decree, or marriage certificate, you must apply in person, and submit the following documents:

  1. Form DS-11: Application for U.S. Passport

NOTE: You must provide a Social Security Number if you been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
